- Maintain patient registries for Quality Measures
- Notify providers of patients who need tests or services through the use of EMR flags and appointment notes
- Create lists of patients who need services for PCP practices to use in outreach efforts
- Provide PCPs with AQC score cards, suggesting opportunities for improvement
- Maintain a clinical data warehouse with multiple sources of data including insurance plan claims data and automatic and manual entry from 5+ EMRs
- Annually report Outcomes Measure data to insurance plans
- Annually submit Quality Measure numerator and denominator appeals including all supporting chart and billing documentation
