  • Maintain patient registries for Quality Measures
  • Notify providers of patients who need tests or services through the use of EMR flags and appointment notes
  • Create lists of patients who need services for PCP practices to use in outreach efforts
  • Provide PCPs with AQC score cards, suggesting opportunities for improvement


  • Maintaining a clinical data warehouse with multiple sources of data including insurance plan claims data and automatic and manual entry from 5+ EMRs
  • Annually report Outcomes Measure data to insurance plans
  • Annually submit Quality Measure numerator and denominator appeals including all supporting chart and billing documentation
