DR. BRENT FRYLING APPOINTED AS REGIONAL MEDICAL DIRECTOR
BILH announced that Dr. Brent Fryling has been selected as a Regional Medical Director for BILH Primary Care – North.
Dr. Fryling is a family medicine physician who has been practicing at, and serving as Medical Director of, BILHPC – Federal Street, Danvers since 2020. Brent received his medical degree from Temple University School of Medicine and completed his residency in family medicine at Lancaster General Hospital in Pennsylvania. He has a special interest in infectious disease and is passionate about primary care and the difference we can make in our patient’s lives.
Dr. Fryling will manage all clinical aspects of his designated regional practices and facilitate daily practice operations with a commitment to providing the highest quality of care and service to patients.

SHIELDS HEALTH LEAVING BCBS NETWORK
Shields Health (Shields) has notified BCBS that they are leaving their network, effective for dates of service on and after December 31, 2023.
“Your Agreement with us requires you to use participating providers whenever possible. If you currently refer our members to Shields for outpatient surgery services or imaging, please start referring them to an in-network provider instead.”
Shields provides imaging services, such as MRI, and PET/CT scans for BCBS members in both freestanding centers and within some hospital locations.

Blue Cross Blue Shield
BCBSMA recently updated the COVID-19 temporary payment policy with the following changes: Added new vaccine administrative code 90480; Added new vaccine codes 91318-91322; Indicated that vaccine code 91304 will be reimbursed by Blue Cross effective October 3, 2023 since it is no longer supplied for free from the state; Indicated deleted vaccine codes and vaccine administrative codes as of November 1, 2023. Are Providers Billing a Covered Diagnosis for These Tests
In September, BCBSMA notified providers of medical policy changes that will take effect on December 1, 2023. BCBSMA is reminding providers of these changes so our members receive the recommended testing: Vitamin B-12 Testing medical policy 061 (new policy); Pathogen Panel Testing medical policy 045 (revised policy). Fallon
Fallon Health extends the Continuity of Care for all Fallon Health-Atrius Health Care Collaborative (FACC) ACO members through February 29, 2024
Fallon Health previously communicated that FACC ACO members have a Continuity of Care period through September 30, 2023. We have made the decision to extend the Continuity of Care for all FACC ACO members through February 29, 2024. This extension will create a new Continuity of Care authorization with a new authorization number.
Please note that standard business processes are still in place for prior authorization requests that are submitted to Fallon Health:
Specialists cannot refer to specialists. Should a specialist need to refer to another specialist, the member’s Primary Care Provider (PCP) is required to request the specialty referral.
Specialists can request a prior authorization for procedures they may be performing.
All prior authorization requests sent to Fallon Health will be reviewed for medical necessity and may be denied if clinical criteria is not met.
Providers should submit new prior authorization requests, with the appropriate clinical information, in ProAuth, Fallon Health’s electronic referral and prior authorization system.

PCP Referrals for MassHealth ACO
In accordance with Managed Care Entity Bulletin 22, we continue to waive the PCP referral requirement for in-network specialty care services for Fallon 365 Care, Berkshire Fallon Health Collaborative, and Fallon Atrius Care Collaborative members. It is Fallon Health’s intention to resume the PCP Referral requirement on January 1, 2024. Please ensure your access to ProAuth is still active.

Payment Policy Update: Nutritional Counseling
Harvard Pilgrim Health Care has also replaced the previous Harvard Pilgrim Nutritional Counseling Payment Policy and Tufts Health Plan Nutritional Counseling Professional Payment Policy with a new, integrated Point32Health Nutritional Counseling Payment Policy. As a reminder, it’s important for medical nutrition therapy services to be reported correctly based upon who is rendering them. As supported by the American Medical Association, CPT code ranges 97802-97804 or G0270-G0271 should only be reported by a medical nutritionist, nutritional professional, or hospital. If a physician or other qualified health care professional provides medical nutrition therapy, an evaluation and management or preventive service code should be selected. MLN Matters Number: MM13017 Revised: Removal of a National Coverage Determination & Expansion of Coverage of Colorectal Cancer Screening
What’s Changed: CMS added clarifying information about the -KX modifier for screening colonoscopy claims in the context of a complete colorectal cancer screening. Substantive changes are in dark red (pages 2-3). Make sure the billing staff knows about: 1) Removal of National Coverage Determination (NCD) 160.22 – Ambulatory Electroencephalographic (EEG) Monitoring; 2) Lowering the minimum age for colorectal cancer screening (CRC) from age 50 to 45 for certain tests; 3) Expanding the definition of CRC screening tests and new billing instructions for colonoscopies under certain scenarios; 4) Medicare manual updates. CMS is reducing the minimum age for coverage for the following CRC screening tests (HCPCS codes G0104, G0106, G0120, G0327, G0328, 81528, and 82270) from 50 years to 45 years of age or older. Marketplace Plan Year 2024 Open Enrollment Fact Sheet
The Marketplace Open Enrollment Period on HealthCare.gov generally runs from November 1 to January 15. Consumers who enroll by midnight December 15 (5 a.m. EST on December 16) can get full-year coverage that starts January 1, 2024. . In 2024, January 15 is a federal holiday; accordingly, consumers will have until midnight on Tuesday, January 16 (5 a.m. EST on January 17) to enroll in coverage. Tufts Health Unify Becomes Tufts Health One Care
Effective Jan. 1, 2024, Tufts Health Unify is changing its name to Tufts Health One Care. By aligning the name with the Massachusetts One Care Program, providers will find it easier to determine which population this plan serves. With a One Care plan, members get the benefits of both Medicare and MassHealth (Medicaid) programs through one health plan. Current Tufts Health Unify members will receive a new member ID card in December to reflect the new product name. The member ID number will remain the same. Tips for Avoiding Unnecessary Denials
CareLink is a national open-access PPO plan that provides both in and out-of-network benefits. CareLink members have access to the Tufts Health Plan provider network in Massachusetts and Rhode Island, and the Cigna provider network in other states. Tufts Health Plans committed to ensuring that providers have an optimal experience when submitting claims for CareLink members. Through ongoing monitoring and review, Tufts Health Plans has compiled a list of reminders to help avoid common unnecessary claim denials and rejections. Fraudulent Calls
NEPHO would like your staff and providers to be aware of fraudulent calls being made to local practices by individuals misrepresenting themselves as Health Plan representatives. As we have seen in the past, individuals are targeting physicians, providers, and the public, in an attempting to extort money and Personal Identifiable Information (PII). As a reminder, the caller may reference a provider’s NPI or state license number, in order to appear more credible and influence the provider or staff to disclose the requested information. The most recent fraudulent incident report was an individual posing as a Health Plan representative, asking for specific providers personal email addresses to verify their information. Please take the steps below in this type of situation, as this is not a typical avenue to verify provider information.
As a best practice, when receiving calls requesting data such as this, it is important for staff to ask for a name, company, call back number and direct email and outline the call will be returned. This gives your practice the ability to confirm the call is not fraudulent, as well as track specific calls coming in. Please share this information with your staff and providers, and ask them to report any questionable calls to your Integrity and Compliance department contact or the Northeast PHO.

OPTUM NEWSLETTER – November- Focus on Diabetes mellitus
Fall Risk Screening
Coding for fall risk screenings varies depending on whether the screening took place during an Annual Wellness Visits (AWV) or a general office visit. There are 2 CPT II codes to choose from 1100F or 1101F. CPT II code 3288F would be added along with either 1100F or 1101F to indicate a fall risk assessment was completed.
- 1100F – Patient screened for future fall risk; documentation of 2 or more falls in the past year or any fall with injury in the past year
- 1101F – Patient screened for future fall risk; documentation of no falls in the past year or only ONE fall without injury in the past year
- 3288F – indicate a fall risk assessment was completed(Should be used with either 1100F or 1101F)
If patients are screened during an AWV, Using additional CPT II performance tracking codes on the claim, such as the 1100F code, will help indicate if the patient has documented falls in the past. To indicate that a patient has had no falls within the past year or one fall without injury, the 1101F code should be used. Additionally, the 3288F code should be used with the functional status CPT II codes (1100F, 1101F) to indicate a fall risk assessment was completed.
If the patient is seen for a general routine encounter, the specific CPT/HCPCS code used would be based on the reason for the visit and time spent with the provider. These codes can range from 99201 to 99350. The CPT II codes 1100F, 1101F and 3288F are suggested in this scenario to ensure the screening “gap” is closed, as this may be a quality performance measure. ICD-10 diagnostic codes are also suggested for any encounter, regardless of AWV, routine or physical exam encounters. These codes may vary and are applicable to the patient’s medical history and current health status at the time of the encounter, such as Z91.81 “At Risk for Falling, History of failing.”
